Step 1 - Preparation Checklist

Please ensure you have reviewed the AWARDS NOMINATION CHECKLIST and have all documentation prepared prior to proceeding with the nomination here. To verify this is complete, please check all boxes to ensure you have all the required information ready for the nomination process. All boxes must be checked complete in order to proceed.


It is strongly advised this nomination is completed on a large screen device such as a desktop or laptop.
Please note your work will NOT be saved if an error occurs. It is strongly advised you type your answers in a Word Doc. or similar format beforehand and then copy and paste to the nomination form to save you time and frustration. 
 

Supporting Documents

You may choose to submit supporting documents that demonstrate the nominee’s achievements. If so please submit the next two (2) list items:

Step 4 - About the Nominee

Tell the jurors who you are and how you know the nominee. It might include:

  • A description of your personal and professional background.
  • Your relationship (if any) with the nominee.
  • The length of time you have known and/or followed the career of the nominee.

Tell the jurors the special qualities that make your nominee worthy of receiving a WEA Arts Leadership Award. It might include:

  • The nominee’s history and background;
  • The nominee’s connection to Windsor and Essex County;
  • A description of the nominee’s achievements; and
  • An explanation as to why these achievements make the nominee the best candidate for the Leadership Award.

Step 5 - Letter of Support

Letter of Support

This letter should be written by someone other than the nominator that can attest to the nominee’s merits and achievements.

Step 6 - Send Nomination and Confirm

Please click the "SEND NOMINATION" button below to submit your Nomination Form to WEA.

After you click the “SEND NOMINATION” button, you should receive notice your nomination was received successfully. You will then be redirected to the final step in the nomination process: Upload Zip Files Via WeTransfer

Please follow these instructions to ensure we receive your files correctly. Additionally, you should receive an email confirmation with links & instructions to upload files as well.

If you receive an error, please note your application has not been submitted successfully and we encourage you to try again.
